KHSAA Physical Forms – Required for CCHS Athletic Teams Participation

All students wanting to participate or tryout for a Campbell County High School Athletic Team must have a completed/current KHSAA Physical Form (GE04) prior to doing so. Please check with your healthcare provider when setting up your child’s sports physical, as some offices require both the GE04 and the PPE01 form. CCHS Athletics in alignment with the KHSAA only requires the GE04 form. See below for links to both forms.
