2014-2015 Sports Physicals

It is that time of year…Fall Sports registration is open. As you know, any student who participates in a sport must have an up to date physical. Campbell County Schools has partnered with NovaCare to make this process easy and affordable.
Here is what you need you know:
2014-2015
CCHS SPORTS PHYSICALS
DATE: THURSDAY, May 22
TIME: Start @ 6:00PM
WHO: Any student(s) wishing to participate in Campbell County Athletics 2014-2015.
WHERE: CCHS Gym
COST: $20.00/per student (CASH OR CHECK, NO INSURANCE ACCEPTED)
NOTES:
1) KHSAA PHYSICAL FORM REQUIRED (Download Here)
2) All physicals MUST be signed by a Parent or Guardian
3) For your convenience, a copy of all physicals will be kept on file in the Athletic office and will apply to all 2014-2015 sports.
