CCYFL: KHSAA Physical Form

All  players must have a physical prior to the Jamboree.  The KHSAA physical form must be used.  You can find the link to the form here:  ms01-total-requiredms01andoptionalppe01.pdf (khsaa.org).  Have your doctor/NP fill it out and hand it to your Team mom.  Thank you.
