Sports Physicals

Sports Physicals will be offered at Campbell Co. High School on Monday June 1st in the main gym starting at 6:00 pm. If your child is needing a physical for sports, band or summer camp, this is a great opportunity to get the physical completed by a trained physician. Physicals are only $20 per person and students of any age/grade level are welcome. NovaCare/UC physicians will be providing the physicals. New KHSAA Physical Forms will be available at the door or by downloading them here, all physicals must be completely filled out and signed by a parent/guardian.
All physicals will be kept on file in the CCHS Athletic Office.
